Ok Kev - I think you are at the point where my favorite suggestion may work for you.
When you get those "thoughts", when that throbbing/pulsing feeling starts up ...
take a deep breath, remind yourself that you've been quit for 30+ days now, and then tell that little demon to take a hike, hit the road, go to he!!, whatever line works for you. My turning point was realizing that :
"This is all bullsh**. It's all a battle in my head and I can control my thoughts and actions if I choose to" AND I swear, from that point on it's been smooth sailing. Not to say I never have smoking thoughts, but I can acknowledge them, tell myself I don't want to go back there and go through this all again and then move on with life.
Try it, getting in control is the difference between easy and hard, at least in my perspective!
